We operate as fee for service and payment is expected at the time of service.

FEE SCHEDULE:

$200 initial visit includes Medical marijuana patient registration application if necessary

$100 follow up visit in 6 months includes Medical marijuana patient registration renewal

$60 interim follow up visits if necessary as needed

Before coming to the office, you must:

1. have one or more of the qualifying conditions in Ohio for Medical marijuana treatment

2. have your physician forward your medical records

3. have payment in the form of cash, check or credit/debit card

*Please note our fees do not include the fee of $50 for the card issued by the State Board of Pharmacy.
The State Board of Pharmacy may discount 50% for veterans and those on SSI or SSDI.
